1. Id line the QB either in the shotgun or the pistol formation. If you line him up under center with only 5 blockers he is gonna get absolutely hammered on the snap. By giving some extra depth that allows time for the blocking to develop and the play to be run
2. Id run left inside receiver on a quick slant , right inside receiver on post route, left outside receiver on a go route and right outside on an out route. the running back would stay at home to block the uncovered end or LB in the blocking and the QB would make a pre snap read of the 4 receivers based on coverage matchup. In TDD's rendition they are playing straight man so the best options are either the inside post or the go route.
