My uncle who is a professor in Tokyo but who lives in Chiba sent me the link to this graphic with the following description:

Don't bother using your browser translation. It is a high res graphic and not text.
I just found out the other day that I'll be travelling through Tokyo on business in July. I am spending a little over 24 hours with my uncle so I should get a better view of life in Japan firsthand.
Sly's Uncle Ralph wrote:This map shows Chiba (the peninsula at the bottom right) up the main Japanese island of Honshu almost to the northern island of Hokkaido. The bars give town/city names (the black box on the left with white letters), then the gray box gives # of dead for that town, the next white box gives # of missing, and the white box on the rt gives # of displaced persons. The 3 red circles in the lower center are around the Fukushima reactors and give the 10 and 20 km radii with instructions for evacuation and the 30 km radius with instructions for staying indoors.
