From the other side of the equation: (Keeping personal feelings to myself)
Assistant coaches are like children, they should be seen but not heard. There is no where in the rule book that says you have to warn someone before an unsportsmanlike technical (The only warning prior to a "T" involves delay of game situations.) Having said that, MOST of the time an official will go over to the head coach and say "Coach, help me out here, keep your bench quiet. " or "Coach, I'll talk and explain things to you all night, but not your assistants." I have handed out "T's" to benches with and without warning. It all depends on the degree of intensity. If an assistant coach stands up and yells something, that is AUTOMATIC! If he/she does it in a way that you can "ignore" it and address it with the coach later, then most officials will. The bottom line is that if the Assistant coach doesn't say anything to the officials they don't have to worry about getting T'd up. Fairly simple.

(BTW, I too was a collegiate assistant coach and VERY well behaved. Now as a head coach........)
As far as officials getting reprimanded it happens all the time. For OBVIOUS reasons, these are not made public. That is accept in the case of the Duke/BC (or was it FSU) game a few years ago when the league was very public in its reprimand to officials. Officials get critiqued every game. They get "reprimanded" for calls they do make and don't make. Most times it is a simple matter of a phone call asking what happened. Sometimes, it is a visit with the "priniciple". The point being, officials don't just hang up the whistles and polyesthers and forget about the game.
