Can anyone who has this game tell me where the hook & ladder play is?

I never found it. The only trick plays I could find were the HB option.

Is it called Hook & ladder? I looked there but only found Statue of Liberty - by the way is the play we should have ran against WM on the 2 pt conversion

try "hook and lateral"
